Become a Patient

If you wish to become a patient, you must first visit one of our Patient Access Representatives. The purpose of this visit is to register you in our system, assist you to know what your health benefits cover and do not cover, assist you to see if you qualify for our sliding discount program for services that your benefits do not cover, orient you on all of our available services and programs, and facilitate your appointment with our providers.

Patients with Commerical Health Insurance

You will need your insurance card and a picture ID. If you do not have a current insurance card, you can provide a letter from your insurance company that contains information about your healthcare coverage.

Patients with Medicaid, CHIP, or Medicare

Parents of children with Medicaid and CHIP will need to provide a valid picture ID (see below for acceptable forms of identification) and the child’s Medicaid/CHIP card:

  • State issued ID or Driver’s License (Out of U.S. is acceptable)
  • School IDs (must include picture and name)
  • Passport IDs
  • Citizenship certificate with picture and name

Persons with Medicaid, CHIP or Medicare who wish to take advantage of services not covered by their insurance plan such as dental services can apply to qualify for our sliding discount program.

Patients who are Self-pay

We highly recommend that you apply for our sliding discount program.

Sliding Discount Program

Centro San Vicente offers a program that reduces the cost of care for uninsured patients or can be applied to insurance deductibles for patients with insurance.  The discounts are based on family size and income that is less than 200% of the Federal Poverty Income Guidelines.

How to Apply

Schedule an appointment with one of our Patient Access Representatives and complete the Sliding Discount Program Application Form.

On the day of your appointment, you will need to provide the following documents:

  • Proof of Identification:
      • For Adults – one (1) identification is required with a picture ID
        • Current Driver’s License
        • State Identification card
        • Legal Resident documents, Passport or Consulate card
      • If no picture ID, bring two (2) of the following:
        • Birth Certificate
        • Social Security card
        • Marriage license
        • Selective Service Card 
      • For Children – bring two (2) of the following:
        • Birth Certificate
        • Social Security card
        • Verification of Birth Facts from Hospital
  • Proof of Income (as applicable):
    • Earned income
      • Pay stubs showing last four (4) weeks of income
      • IRS Tax record transcript (request online @ www.irs.gov or 1-800-908-9946)
      • Statement from employer with company’s information
    • Unearned income
      • Recent TANF letter from Texas Department of Human Services
      • Social Security Supplement from Social Security Administration
      • Child support award letter from Attorney General
      • Retirement Pension award letter
      • Veteran’s pension letter
      • Unemployment award letter from Texas Workforce Commission
      • Workers’ compensation letter
    • Self-employment
      • Declaration letter for last 2 months
      • IRS Tax record transcript (request online @ www.irs.gov or 1-800-908-9946)
  • Proof of Residency:
    • Utility bills – most recent
      • Electric bill
      • Water bill
      • Natural Gas bill
    • Food stamp letter
    • Lease from dwelling (house, apartment, etc.)
    • Letter of residency verification

If patients wish to maintain eligibility under the Sliding Discount Program, the eligibility information must be updated every year or whenever there are changes in household size, household income or if someone in the household obtains or loses medical coverage. If the eligibility information is not updated, your account will be charged at the full fee.

Co-pays (patients with Commercial Health Insurance) & Nominal Fees (Self-pay patients)

Co-pays and nominal fees are required when you check-in for your visit.  Any additional charges will be collected when you check-out.  For your convenience, we accept Cash, Checks, Debit and Credit Cards. 

Payment Arrangements

No patient will be refused services due to the inability to pay. Financial arrangements can be made with the Billing Department, please call (915) 859-7545 extension 1227.

Patient Rights and Responsibilities

At the time of your appointment, our Patient Access Representatives will require you to read and sign patient rights and responsibilities. A copy of this document will be provided to the patient.
