To register, bring proof of residence and insurance if applicable. Discounts are based on Federal Poverty Income Level guidelines and must be renewed annually.

To Register or Renew Membership, Please Provide:
Proof of residence If no utility bill, a letter from the homeowner and their utility bill
Rent receipt (if no utility bill)
Recent check stubs (one month's worth)
Insurance information (Medicaid, Medicare, CHIP, etc.)
Centro San Vicente Account card (for membership renewal)
Financial Services
We offer various programs based on income and family size, each with its co-pay. A sliding-scale discount program is available for those who don't qualify for other programs. Additional fees may apply for lab, X-ray, or pharmacy services.
Medicaid, Medicare, and Insurances
We accept most Medicaid Managed Care Health Plans, CHIP, Medicare, and private insurances. Medicare patients are responsible for the 20% uncovered charges. Medicaid eligibility assistance is available.
Payment Arrangements
No patient is denied services due to an inability to pay. Payment arrangements can be made for unpaid balances.
